Understanding Canker Sores: What They Are and Why They Form

A canker sore is a small, painful ulcer that develops inside your mouth. Unlike cold sores, which appear on the outside of your lips and are caused by the herpes simplex virus, canker sores occur only inside the mouth and are not contagious. They typically appear as round or oval lesions with a white or yellowish center surrounded by a bright red, inflamed border. Most canker sores develop on the soft tissues inside your mouth, including the inside of your cheeks, lips, tongue, and the base of your gums.

Canker sores develop through multiple causes. One common cause is minor mouth injuries from dental work, aggressive brushing, accidental cheek biting, or sharp foods like tortilla chips or hard candies. Research shows that about 20% of the population experiences canker sores regularly, with women reporting slightly higher rates than men. Another significant cause is nutritional deficiency—studies indicate that people lacking vitamin B12, zinc, folate, or iron are more prone to developing these sores. Certain foods can also trigger their formation, particularly acidic or spicy items like citrus fruits, tomatoes, pineapple, and hot peppers.

Other contributing factors include stress and lack of sleep, which can suppress your immune system and make mouth tissues more vulnerable to ulceration. Some people develop canker sores as a reaction to sodium lauryl sulfate, a common ingredient in many toothpastes. Certain medical conditions, such as celiac disease, Crohn's disease, or aphthous stomatitis (a condition characterized by recurring mouth ulcers), can make canker sores more frequent. Hormonal changes during menstrual cycles also trigger canker sores in some women.

Most canker sores are small (less than one inch across) and heal within one to two weeks without leaving scars. However, larger canker sores called major aphthous ulcers can persist for several weeks and may require professional medical attention. Understanding what causes your specific canker sore helps you select the most appropriate treatment and prevention strategy.

Practical Takeaway: Keep a small notebook tracking when canker sores appear, what you ate before they developed, and any stress or sleep changes. This record helps identify your personal triggers and allows you to avoid them in the future.

Over-the-Counter Products and Topical Treatments

Many over-the-counter products can reduce canker sore pain and potentially speed healing. Topical anesthetics containing benzocaine (such as Orajel or Anbesol) numb the sore temporarily, typically providing relief for 1 to 2 hours. These products work by blocking nerve signals in the affected area. While they don't treat the underlying sore, they make eating and speaking more bearable during the healing process. Apply these products directly to the canker sore following the package directions, usually every few hours as needed.

Antimicrobial mouthwashes containing hydrogen peroxide or chlorhexidine may help prevent secondary bacterial infections while keeping the area clean. Products like Peroxyl Oral Rinse contain hydrogen peroxide at a concentration designed specifically for mouth use. These rinses work by releasing oxygen bubbles that mechanically clean the sore. Clinical studies suggest that antimicrobial rinses may slightly reduce healing time when used consistently, though the difference is often modest—sometimes just a day or two. Use these rinses 2 to 3 times daily, especially after meals.

Topical corticosteroid products, available both over-the-counter and by prescription, can reduce inflammation and pain. Over-the-counter hydrocortisone cream (1%) can be applied carefully to canker sores, though prescription-strength products like triamcinolone are more effective for larger or particularly painful sores. These work by suppressing the inflammatory response that makes the sore painful and slows healing. Most people see reduced pain within 24 hours of starting corticosteroid treatment.

Oral patches and protective barriers (such as Canker Cover or similar products) physically shield the sore from friction and irritants while delivering medication. These patches adhere to the canker sore and create a protective seal that reduces contact with saliva, food, and bacteria. Some formulations include topical anesthetics or healing agents. Research shows that protective barriers may reduce pain by 50% or more and potentially shorten healing time by 1 to 3 days.

Antimicrobial gels containing silver ions or iodine compounds have shown promise in research studies for reducing healing time. These gels kill bacteria that can cause secondary infections and may reduce inflammation. Products containing povidone iodine (available at most pharmacies) can be applied 2 to 3 times daily, though you should avoid these if you have an iodine allergy.

Practical Takeaway: Start with a topical anesthetic to manage pain while eating and speaking, combine it with an antimicrobial rinse for cleansing, and consider adding a protective patch if the sore is in a high-friction area. This multi-pronged approach addresses different aspects of canker sore discomfort.

Natural and Home Remedies with Scientific Support

Salt water rinses represent one of the oldest and most well-researched home treatments for canker sores. Mix 1/4 to 1/2 teaspoon of salt in 8 ounces of warm water and rinse your mouth for 30 seconds, 2 to 3 times daily, especially after meals. Salt water works by drawing out fluid from the inflamed tissue (osmosis), reducing swelling and creating an environment where bacteria struggle to survive. While salt water doesn't directly speed healing, it reduces inflammation and pain while keeping the area clean. The burning sensation you might feel initially comes from the salt water reaching raw tissue, but this typically subsides after a few rinses.

Honey has demonstrated antimicrobial and anti-inflammatory properties in multiple clinical studies. Raw honey, which contains enzymes and compounds like hydrogen peroxide, may be more effective than processed honey. Apply a small amount of raw honey directly to the canker sore 3 to 4 times daily. Research from Kuwait University found that honey applied to mouth ulcers reduced pain significantly and accelerated healing compared to topical corticosteroids in some cases. The thickness of honey also provides a protective barrier against irritating foods and bacteria.

Milk of magnesia, commonly used for upset stomachs, can coat and protect canker sores. Its alkaline properties may neutralize acids that irritate the sore. Apply milk of magnesia directly to the canker sore 3 to 4 times daily using a clean cotton swab. Some people find this particularly helpful because it adheres better than liquids and creates a longer-lasting protective coating.

Baking soda rinses combine antimicrobial properties with alkalinity. Mix 1/2 teaspoon of baking soda in 8 ounces of water and rinse for 30 seconds. The alkaline environment baking soda creates is unfavorable for bacterial growth, and the rinse helps remove food particles and dead tissue. You can also make a paste by mixing baking soda with water and applying it directly to the sore. Research suggests that alkaline conditions may speed healing and reduce discomfort.

Ice provides temporary pain relief through numbing and reducing inflammation. Apply ice directly to the canker sore for 5 to 10 minutes at a time, several times daily. This approach is particularly useful before eating or drinking when you anticipate pain. Some people find ice lollipops or ice chips helpful because they provide both the numbing effect and maintain hydration.

Chamomile tea bags contain compounds with anti-inflammatory and antimicrobial properties. Steep a chamomile tea bag in hot water, allow it to cool slightly, then place it directly against the canker sore for 5 to 10 minutes. The combination of gentle heat and chamomile's active compounds may reduce inflammation and promote healing. Repeat this 3 to 4 times daily for best results.

Practical Takeaway: Begin with salt water rinses as your foundation treatment since they're inexpensive, have no side effects, and provide immediate cleaning. Add honey or milk of magnesia if you want an extra protective barrier, and use ice when you need quick pain relief before meals.
