Learn About Recognizing Concussion Signs and Symptoms

What Is a Concussion and How Does It Happen

A concussion is a type of traumatic brain injury caused by a bump, blow, or jolt to the head that changes how the brain normally works. It can also happen when a hit to the body causes the head and brain to move rapidly back and forth inside the skull. When this happens, brain cells are stretched and damaged, and chemical changes occur in the brain. These changes can affect how a person thinks, moves, and feels for a period of time.

According to the Centers for Disease Control and Prevention (CDC), approximately 1 million emergency department visits per year involve traumatic brain injuries, with concussions being the most common type. In youth sports alone, there are an estimated 300,000 sports-related concussions annually in the United States. However, the actual number may be higher because many concussions go unreported, especially in young people who may not realize they've had one.

Concussions can occur in many different situations. Common causes include falls, vehicle accidents, assaults, and sports injuries. Falls are the leading cause of concussions overall, particularly in older adults and young children. In sports, concussions happen in both contact sports like football and hockey, and non-contact sports like soccer and basketball. Even a relatively minor-seeming impact can cause a concussion—a person doesn't have to lose consciousness for a concussion to occur, and in fact, most concussions happen without loss of consciousness.

Understanding how concussions happen is important because it helps people recognize situations where one might occur. A person can get a concussion from being tackled in sports, hitting their head on a low doorway, being in a car accident, falling off a ladder, or being struck in the head during an altercation. The force required to cause a concussion varies from person to person. Some people may be more vulnerable to concussions than others, including those who have had previous concussions.

Practical Takeaway: Knowing that concussions can result from various impacts—not just obvious collisions—helps people stay alert to potential injuries in everyday situations and sports activities.

Immediate Signs of Concussion Appearing Right After Injury

Some signs of a concussion appear immediately or within minutes of the injury. These are often called acute or immediate symptoms. Being able to recognize these signs is crucial because seeking medical attention right away can make a significant difference in recovery. If someone has experienced a head injury, watching for these immediate signs can help determine whether they need medical evaluation.

Physical signs that may appear immediately include loss of consciousness, though this happens in only about 10% of concussions. More common immediate physical signs include appearing dazed or confused, moving clumsily or with poor coordination, answering questions slowly, forgetting instructions, or showing mood changes like irritability or emotional reactions that seem out of proportion to the situation. Some people may complain of a headache, neck pain, or nausea right after the injury.

One important immediate sign is confusion about what just happened. A person who has sustained a concussion might not know what play is happening in a game, forget their assignment at work, or be disoriented about time and place. They might repeat the same question multiple times or seem unable to follow a conversation. Coaches and athletic trainers are trained to watch for athletes who seem stunned, don't know what down it is in football, or can't remember the opposing team's name.

Balance and coordination problems are another immediate sign. A person might stagger or stumble when trying to walk, seem clumsy when moving, or have difficulty with fine motor tasks like catching a ball or writing. Vision changes can also occur, such as blurred vision or sensitivity to light. Some people report feeling off-balance or dizzy. These signs suggest the injury has affected the part of the brain that controls coordination and balance.

Speech changes can sometimes be noticed immediately as well. A person might slur their words, speak more slowly than usual, or have difficulty finding the right words. Their speech pattern might sound different to someone who knows them well. These changes, combined with other signs, can indicate a concussion has occurred.

Practical Takeaway: Learning to spot immediate signs like confusion, clumsiness, or dazed behavior after a head injury makes it possible to get medical evaluation quickly, which is important for proper recovery.

Delayed Symptoms That Develop Hours or Days After Injury

Not all concussion symptoms appear immediately. Many people develop symptoms hours or even days after a head injury. These delayed symptoms are important to recognize because people might not initially realize they have a concussion, especially if the initial impact didn't seem severe. Someone might feel fine right after hitting their head but develop concerning symptoms later.

Headaches are among the most common delayed symptoms and may develop or worsen over hours following the injury. Some people describe it as a pressure sensation, while others report throbbing or persistent aching. Sleep disturbances are also common delayed symptoms. A person might have difficulty falling asleep, wake up frequently during the night, or find they need more sleep than usual. Some people become drowsy during the day despite getting what seems like adequate sleep.

Memory and concentration problems frequently develop after a concussion. A person might struggle to remember recent events, have difficulty concentrating on work or schoolwork, or feel mentally foggy. This symptom is sometimes called "brain fog" and can persist for days or weeks. A student might report difficulty focusing during classes or retaining information from reading. An adult might struggle with work tasks that require sustained attention or memory.

Sensitivity to light and noise can develop as a delayed symptom. Bright lights might feel uncomfortable or cause headaches to worsen, and normal sounds might feel too loud. This sensitivity is called photophobia and phonophobia. Someone might find themselves wanting to stay in dimly lit, quiet environments and feeling irritated by stimulation they normally would tolerate.

Mood and behavior changes can also emerge over time after a concussion. A person might become more irritable, anxious, or sad. They might experience mood swings or feel emotional over things that wouldn't normally upset them. Some people report feeling depressed or having difficulty managing stress. These emotional changes can be confusing to the person experiencing them and to their family members.

Nausea and dizziness that weren't present immediately may develop hours later. A person might feel off-balance when walking, experience vertigo, or feel queasy. These symptoms can come and go throughout the day.

Practical Takeaway: Since concussion symptoms can develop hours or days after an injury, continued observation is necessary even if someone seems fine immediately after a head injury.

Differences Between Concussion Symptoms and Normal Injury Recovery

Sometimes it can be difficult to determine whether symptoms after a head injury are signs of a concussion or simply normal recovery from trauma. Understanding the differences helps in deciding whether medical evaluation is needed. A person might have a minor bump or bruise on the head without a concussion, while another person might have a concussion without obvious external injuries.

A key difference is that concussion symptoms relate to changes in how the brain is functioning, not just to the presence of an external wound. A person might have a visible cut or bruise on their head but no concussion, while another person might have a concussion with no visible head injury at all. The symptom pattern of a concussion typically includes cognitive changes, balance problems, or emotional changes—not just pain at the injury site.

Timing and pattern of symptoms differ in concussions. A simple bump or bruise might cause immediate pain that gradually improves as the swelling goes down. Concussion symptoms may worsen over the first 24 to 48 hours, remain stable for a time, and then gradually improve. If someone is feeling progressively worse rather than better over the first few days after an injury, this may suggest a concussion.

The type of symptoms also matters. A mild concussion might involve only one or two symptoms, while more significant concussions involve multiple symptoms affecting different areas—such as both balance problems and memory difficulties. The combination of multiple symptoms is more suggestive of a concussion than a single symptom alone.

Recovery trajectories also differ. Most uncomplicated minor head injuries with no concussion improve within a few days. Concussions often take longer to resolve, with recovery typically taking 7 to 10 days for adults and sometimes longer for children and teenagers. If symptoms are still present after a week, medical evaluation becomes increasingly important.
